I understand by submitting this application, I give Collar of Hope permission to contact my landlord if applicable, permission to contact my Veterinary reference and permission to contact my personal references listed.
If you become interested in adopting a foster, you must notify the rescue. If an applicant for the animal is approved prior to your notification, you will have to wait until a decision is made on the applicant's adoption before you will be considered.
Collar of Hope will need regular updates and information about the foster dog(s) to best place them into a permanent home.

I understand that any misrepresentation of the above information authorizes Collar of Hope to deny application, and/or reclaim the pet that is in my home. I understand Collar of Hope will furnish all known information about a dog but as dogs in different environments and with different handlers dogs can react or behave differently. I understand completely that it is our responsibility to evaluate the dog for myself before agreeing to foster it. Collar of Hope is in no way liable or responsible for any damage, accident or injury resulting from the placement of a dog in my household.
